Upload Vaccination Records

Thank you for keeping your pet’s records up to date!
Please upload a clear copy or photo of your dog’s current vaccination records below.

Required Vaccines:
• Rabies
• Distemper/Parvo (DHPP or DAPP)
• Bordetella (kennel cough)

If your vet is sending records directly, please have them email them to greencovekennel@gmail.com.

Once we receive your records, we’ll review them and send a confirmation email to verify your file is up to date.

Notes:

• Records must list your dog’s name, vaccine type, and the administration/expiration dates.
• We recommend submitting updates at least 48 hours before check-in to allow the vaccines time to take effect.
• Accepted file types: PDF, JPG, or PNG.
